I hereby allow my child to participate in VBS at Mount Hermon Baptist Church and all of its activities. I hereby agree to accept and assume full responsibility for risks, including physical illness and injury associated with participation in this event. I hereby agree to indemnify, defend, and hold harmless Mount Hermon Baptist Church and each of its employees, officers, representatives, and volunteers against any liability, cost, loss, claims, and actions, including negligence based upon or sustained in connection with participation. Should an emergency arise due to physical illness or injury, I hereby request and authorize Mount Hermon Baptist Church and their leaders to seek whatever medical care is necessary and advisable. I agree to indemnify, defend, and hold harmless Mount Hermon Baptist Church and the leaders in the exercise of this authority I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my child for physical illness or injury sustained in association with participation in this VBS event.
I am the parent or legal guardian of the minor registered above for VBS. I have read and understand this Agreement in its entirety and that it relates to surrendering valuable legal right of the minor and myself. I agree to be bound by the terms of the Agreement.
